Scottish NHS patients can now sign GP17 forms digitally! 🏴󠁧󠁢󠁳󠁣󠁴󠁿

No more complicated workflows or fiddly paperwork - for NHS Scotland practices, your patients can quickly and easily complete their GP17 forms on a tablet before and after their appointments, saving them time and reducing your workload.

How can my patients complete GP17 forms digitally?

You just need Dentally Portal's Concierge!

If you're already a Concierge user - then you're ready to go! No further action is needed - any eligible patients will see the GP17 task when they log into Concierge in your practice, and can complete their forms digitally.

Haven't tried Concierge yet? It's easy to get started - you'll just need to complete a couple of steps so your patients can sign GP17 forms digitally.

  • Check here to see if you're eligible to use Concierge for free, or whether you need to upgrade your Dentally package.
  • Once it's turned on for your practice, go to the Dentally Portal admin dashboard and click on the 'Devices' section.
  • Follow the steps to pair your tablet device.
  • Then you're all set! Your patients are ready to start completing their GP17 forms on your tablet in the practice.

💡 Top tip: If a patient meets the NHS Scotland criteria and a GP17 form is required, you'll be prompted in the visit panel.

Once you're set up, patients can quickly complete Parts 1 and 2 of their form on a tablet device (read our detailed guide here on how this will look for the patient). Once they've completed one or both parts of the GP17 form, you will be able to view them in the patient's record, in the NHS tab (under 'Exemptions').
